In particular, perioperative analyzer monitoring is critical for diagnosing potential causes of hemorrhage, guiding hemostatic therapies, and predicting the risk of bleeding during subsequent surgical procedures. Hemostasis (the process by which blood changes from a fluid to a solid state) is a complicated mechanism that involves many steps. In general, intact blood vessels contain endothelial cells that prevent clot formation through the action of tissue plasminogen factor, inactivating thrombin, and ADP, among other mechanisms.

Recent years have seen a significant increase in the number of Coagulation Analyzer screening tests that have been requested. Consequently, the efficiency of automated coagulation testing has increased, allowing for the development of high-throughput, accurate, and precise analyzers that require little or no human intervention in the measurement process. The first coagulation analyzers were mechanically operated, with a hook used to detect a clot in the cuvette as the only means of detection. Clotting factors are now detected simultaneously using clotting, colorimetric, and immuno principles, as opposed to the previous method of sequential detection.

Currently available technologies include automated platelet function analyzers, flow cytometers, polymerase chain reaction (PCR), and microarrays. The combination of all of these technologies eliminates the need for pre- and post-analytical handling, resulting in improved accuracy and increased productivity.

The PT/INR test is a straightforward and widely used procedure. When a sample is mixed with thromboplastin, a prothrombin time (PT) test (traditional) demonstrates activation through mechanical or optical means, respectively. A clot (both extensive and local) is detected when the thrombin enzyme is activated and converts fibrinogen to fibrin. The results of the test are reported as an International Normalized Ratio (INR) within seconds of the test being completed. The use of an INR Coagulation Analyzer is then recommended during the monitoring of oral anticoagulant therapy. When calculating INR, take the ratio of patient prothrombin time (s) ISI to mean normal prothrombin time into consideration.

While PT/INR tests are routine, the value of these tests in the acute perioperative setting has been questioned due to a delay of nearly an hour between sampling and the receipt of results. The tests are performed on plasma rather than whole blood, and there is no information available on platelet function. In addition, the assays are performed at 37°C rather than at the body temperature of the patient.

The i-STAT PT/INR in vitro diagnostic test for quantitative monitoring of oral anticoagulant therapy monitoring is one of the point-of-care products offered by coagulation machine Abbott Diagnostics, Abbott Park, Illinois, among other things. While it is limited in its ability to evaluate individual factor deficiencies, when used in conjunction with the i-STAT Portable Clinical Analyzer and the i-STAT 1 Analyzer, it is effective in identifying the coagulation end point. The cartridge is intended to accept sample volumes ranging from 20 to 45 microliters.

Other products that are related to coagulation system analysis are focused primarily on hemostasis and provide not only PT/INR data but also information on the coagulation factors that are involved. Beckman Coulter, Brea, Calif., in collaboration with Instrumentation Laboratory (IL), Lexington, Mass., recently released its latest hemostasis analyzer, the ACL TOP 500 CTS (closed tube sampling) for hemostasis. The analyzer is intended for use in mid-to-high-volume laboratories with more than 800 samples tested per run and 240 test results per hour, according to the company's press release. In this analyzer, a prothrombin time of approximately 3 minutes is achieved, and other Coagulation Analyzer tests such as clotting, immunoturbidimetric, and chromogenic tests can also be completed in this analyzer.
